Tummy Time and Flat Spots: A Parent's Guide to Your Baby's Head Shape

Published October 22nd, 2026 by Camarata Chiropractic

Every new parent eventually notices it.

You are cuddling your baby, running your hand over the back of their head, and you feel it: a flat spot. Maybe one side looks a little flatter than the other. Maybe one ear seems slightly forward.

It is easy to panic. But flat spots are very common, especially in the first few months, and in most cases they respond well to simple, early changes.

Why Flat Spots Happen

A newborn's skull is soft and moldable by design. That flexibility helps during birth and allows for rapid brain growth in the first year.

But it also means that if your baby spends a lot of time with pressure on the same area of the head, that area can flatten.

The medical term is positional plagiocephaly when one side is flatter, or brachycephaly when the back of the head is flat across.

Why It Is More Common Today

Since the "Back to Sleep" campaign encouraged parents to place babies on their backs to sleep, rates of sudden infant death have dropped significantly. That is a huge success, and back sleeping remains the right recommendation.

The trade-off is that babies spend a lot more time on the backs of their heads, so flat spots have become more common. Car seats, swings, bouncers, and loungers add even more time in the same position.

The answer is not to change sleep position. It is to balance it with plenty of supervised awake time in other positions.

Why Some Babies Favor One Side

Many babies with flat spots prefer to turn their head in one direction. That preference can come from:

  • Positioning in the womb, especially in the final weeks
  • A difficult or assisted delivery
  • Tight neck muscles, sometimes diagnosed as torticollis
  • Habit, such as always looking toward the room or a window from the crib

When a baby always turns the same way, pressure keeps landing on the same spot, and the flatness can progress.

Signs to Watch For

  • A flat area on the back or one side of the head
  • One ear positioned further forward than the other
  • Asymmetry in the forehead or face
  • Your baby strongly preferring to look one direction
  • Difficulty turning the head fully to one side
  • Head tilting to one side
  • Trouble breastfeeding on one side

Tummy Time: Your Best Tool

Tummy time is supervised time when your awake baby is placed on their belly. It takes pressure off the back of the head and builds the neck, shoulder, and core strength babies need for rolling, crawling, and sitting.

Tips for making tummy time work:

  • Start early. Even newborns can do a few minutes on your chest.
  • Short and frequent beats long and miserable. Several sessions of a few minutes, building toward a total of an hour or more spread across the day by around three months.
  • Get down at their level. Your face is the best toy there is.
  • Use a rolled towel under the chest if your baby struggles.
  • Try different positions: on your chest, across your lap, or over a nursing pillow.
  • Time it well. After a nap and diaper change, not right after a full feeding.

Lots of babies protest at first. That is normal. It gets easier as they get stronger.

Other Positioning Tips

  • Alternate which end of the crib your baby's head is at so they turn different ways to see the room
  • Switch the arm you hold them in during feeding
  • Limit time in car seats, swings, and bouncers when not traveling
  • Carry your baby upright or in a carrier more often
  • Encourage turning toward the non-preferred side with toys and your voice

How Pediatric Chiropractic Can Help

Infant chiropractic care looks nothing like adult care. The pressure we use is very light, often about the amount you would use to test the ripeness of a tomato.

For babies with flat spots or a strong head preference, our focus is on:

  • Evaluating neck and upper back motion to see if restriction is making it hard to turn
  • Gentle techniques to support comfortable, even movement
  • Coaching parents on positioning, carrying, and tummy time strategies
  • Monitoring progress over time
  • Coordinating with your pediatrician, and referring when needed

Many parents tell us their baby seems more comfortable during tummy time and feeding once neck motion improves.

When to Talk With Your Pediatrician

  • Flatness that is not improving after a few weeks of positioning changes
  • A head that seems unusually shaped from birth, or a hard ridge along the skull
  • Significant facial asymmetry
  • Delays in reaching milestones
  • A baby who seems to be in pain or is unusually irritable

Some moderate to severe cases benefit from a helmet, which works best when started within the first several months. Your pediatrician can help guide that decision.

Common Questions

How much tummy time does my baby need?

Start with a few minutes several times a day and build toward an hour or more total spread across the day by about three months. Short, happy sessions beat long, frustrated ones.

Does my baby need an adjustment if their head is flat?

Not every baby does. We evaluate neck and upper back motion, and if restriction is limiting how your baby turns, gentle care can help.

The Bottom Line for Parents

Flat spots are common, they are usually not dangerous, and they often improve with simple, consistent changes. Start early, keep tummy time positive, and do not hesitate to ask questions.
