SoftWave for Shin Splints: Help for Cross Country and Fall Runners

Ask any cross country coach what the most common complaint is in the first month of the season, and you will hear the same two words.

Shin splints.

It starts as a dull ache along the inside of the shinbone. At first, it fades once the runner warms up. Then it hangs around longer after each run. Before long, it hurts to walk down the stairs.

Shin splints are common, but they are not something runners should just "toughen up" through, because left alone they can turn into something more serious.

What Shin Splints Actually Are

The medical term is medial tibial stress syndrome. It describes pain along the inner border of the tibia, usually in the lower half of the shin.

The pain comes from stress on the bone and the tissue that attaches to it. Your calf and deep lower leg muscles anchor along that inner edge of the shin, and every stride pulls on that attachment while the bone itself absorbs impact.

When the stress outpaces the bone's ability to adapt and remodel, it gets irritated.

Why Fall Runners Get Them

  • Sudden jumps in mileage at the start of the season, especially after a light summer
  • Hard surfaces like roads and sidewalks
  • Hills and uneven cross country courses
  • Worn-out shoes or switching to spikes too quickly
  • Weak calves, hips, and feet
  • Overstriding, landing with the foot far out in front of the body
  • Low energy availability, not eating enough to support training, which is common in teen distance runners

Shin Splints vs a Stress Fracture

This is the most important part of this article.

Shin splints and stress fractures exist on the same spectrum. Ignore shin splints long enough and the bone can develop a stress reaction or stress fracture, which can mean weeks to months off.

Signs that point toward a possible stress fracture:

  • Pain in one specific, small spot you can point to with one finger
  • Pain that gets worse during the run instead of easing
  • Pain at rest or at night
  • Pain with hopping on one leg
  • Swelling over the bone

If any of those apply, stop running and get evaluated. Imaging is often needed.

  • Reduce volume temporarily rather than stopping completely, if pain allows
  • Run on softer surfaces like grass and trails when possible
  • Cross-train with biking or swimming to maintain fitness

  • Rebuild mileage gradually, roughly 10 percent per week

A Note for Parents of Teen Runners

Teen distance runners, especially girls, are at higher risk for bone stress injuries when they are not eating enough to fuel their training. Irregular or missed periods can be a warning sign.

If your runner has repeated shin pain or stress injuries, it is worth looking at nutrition and talking with their doctor. Healthy bones need fuel.

Prevention for Next Season

  • Keep some running in the summer so fall is not a shock
  • Build calf and foot strength year-round
  • Replace shoes every 300 to 500 miles
  • Prioritize sleep and nutrition
  • Speak up early when something feels off

Common Questions

Should my runner take time off completely?

Not always. If pain is mild and does not get worse during runs, reducing volume and running on softer surfaces may be enough. Pain at rest, at night, or in one pinpoint spot is different and needs evaluation before more running.

Do orthotics help shin splints?

Some runners benefit, especially if they have significant foot mechanics issues, but orthotics are not a cure-all. Strength, load management, and good footwear usually matter more.

A suspected stress fracture needs proper evaluation and imaging first. Treatment decisions come after we know what we are dealing with.

Are shin splints more common in girls?

Female runners do have a higher rate of bone stress injuries, often related to fueling and bone health. Paying attention to nutrition, menstrual health, and gradual training increases helps protect them.
