Headaches and Migraines: The Neck Connection Most People Miss

Most people treat headaches as a head problem.

It hurts in your head, so that must be where the trouble is.

Reach for the medicine cabinet, wait it out, get on with your day.

But for a large share of the headaches we see in our office, the actual source is sitting a few inches lower.

It is your neck.

The Anatomy Nobody Explains

The top of your neck is a busy neighborhood.

The upper cervical vertebrae, the muscles at the base of your skull, and the nerves that exit that region are all packed into a small space.

Several of those nerves travel up into the scalp, behind the eyes, and across the temples.

Here is the key part.

The nerve pathways coming from your upper neck converge with the pathways that carry sensation from your head and face.

Your brain receives signals from both through overlapping circuitry.

Which means irritation originating in your neck can genuinely be perceived as pain in your head.

You are not imagining it. Your brain is simply reading a real signal from a slightly different address.

Common Headache Types With a Neck Component

Tension-type headaches

The most common type by far. Often described as a band of pressure around the head, or a dull ache at the temples and base of the skull.

Frequently tied to sustained muscle tension in the neck and upper back.

Cervicogenic headaches

These originate directly from structures in the neck.

They tend to be one-sided, often start at the base of the skull and travel forward, and are commonly aggravated by neck movement or sustained postures.

People with these often report neck stiffness alongside the headache.

Migraines

Migraine is a distinct neurological condition, and we are careful not to oversimplify it.

That said, many migraine sufferers report neck pain and stiffness before or during an attack, and neck tension is a commonly reported trigger.

Addressing neck mechanics is not a cure for migraine, but for many people it is a meaningful piece of the puzzle.

Why Modern Life Makes This Worse

Your head weighs roughly ten to twelve pounds when balanced properly over your shoulders.

Tilt it forward to look at a phone or a low screen, and the effective load on the neck muscles increases substantially.

Now hold that for hours a day, five days a week, for years.

The muscles at the base of the skull stay contracted. Joint motion in the upper neck decreases. Tissue adapts to a position it was never meant to hold.

Then people wonder why they get a headache every afternoon around three o'clock.

Common contributors we see

  • Long hours at a desk or laptop

  • Phone use with the head dropped forward

  • Sleeping on your stomach with the head rotated all night

  • An old whiplash or sports injury that never fully resolved

  • Chronic stress, which parks tension in the neck and shoulders

  • Poor pillow support

How We Approach Headaches

The first thing we do is figure out what kind of headache we are dealing with.

That means an actual history and examination, not a guess.

We want to know when they started, what they feel like, where they are located, what triggers them, what relieves them, and what else is going on in your life and health.

Then we assess the neck directly. How well does it move? Where is motion restricted? Which tissues are tender? Is there a pattern that lines up with your symptoms?

If the mechanics of your neck line up with your headache pattern, that is very useful information.

What treatment typically involves

  • Chiropractic adjustments to restore motion in restricted segments of the neck and upper back

  • Work on the surrounding soft tissue

  • Specific guidance on the postures and habits feeding the problem

  • Simple movement and strengthening recommendations you can actually keep up with

What You Can Change Today

Some of the most effective changes cost nothing.

  • Raise your screen. The top of your monitor should be near eye level. A stack of books works fine.

  • Lift your phone. Bring it up to you rather than dropping your head down to it.

  • Move every half hour. Stand, roll your shoulders, look around. Static loading is the enemy.

  • Fix your pillow. It should keep your neck roughly neutral, not propped up or dropped down.

  • Stop sleeping on your stomach. It requires rotating your neck for hours at a time.

  • Hydrate. Dehydration is a genuinely common headache trigger and an easy one to rule out.

Keep a Simple Log

If headaches are a regular part of your life, track them for two weeks.

Note the date, time, where the pain is, how bad it is, what you were doing beforehand, how you slept, and what you ate.

Patterns show up quickly, and they are extremely useful when we sit down together.

When to Get Checked Sooner

Most headaches are not dangerous, but some warrant prompt medical evaluation.

  • A sudden, severe headache unlike anything you have experienced

  • Headache with fever, stiff neck, confusion, or vision changes

  • Headache following a head injury

  • A clear change in pattern, frequency, or severity

  • Headache with weakness, numbness, or difficulty speaking

If any of those apply, seek medical care right away. We would tell you the same thing in the office.

You Do Not Have to Just Live With Them

The thing we hear most often is some version of "I have just always gotten headaches."

Often that has been true for years, and nobody ever looked at the neck.
