Pain Awareness Month: Five Pain Myths Worth Letting Go

September is Pain Awareness Month, and I want to use it to clear up a few things.
Every week, people sit across from me and explain their pain using ideas they picked up somewhere along the way. A comment from a relative. A line from a commercial. Something they read at two in the morning when their back would not let them sleep.
Some of those ideas are harmless. Others keep people stuck for years.
Here are five pain myths I would love for you to let go of this month.
Myth #1: If It Hurts, Something Must Be Damaged
Pain is real. Every bit of it. But pain is not a perfect damage meter.
Your nervous system produces pain as a protective signal. It weighs what is happening in your tissues, but it also weighs your sleep, your stress, your past injuries, and how threatened your brain feels about a body part.
That is why two people can have nearly identical X-rays and completely different experiences. One person has a disc bulge and no symptoms at all. Another has a "normal" scan and cannot get out of a chair.
This matters because people who believe every twinge means more damage tend to stop moving. And as we will see, that usually makes things worse.
Myth #2: Rest Is the Best Medicine
Short-term rest has its place, especially in the first day or two after an acute injury.
Long-term rest is a different story.
Joints are nourished by movement. Muscles weaken quickly when they are not used. Stiffness builds on itself. The pattern we see over and over looks like this:
It hurts, so I move less. I move less, so I get stiffer. I get stiffer, so it hurts more.
Breaking that loop almost always involves restoring movement gradually and confidently, not waiting for pain to vanish before you do anything.
The better question is not "what hurts?" It is "what can I safely do today?" Start there and build.
Myth #3: Pain Is Just Part of Getting Older
Age changes tissue. There is no point pretending otherwise.
But I see plenty of 70-year-olds who move well and plenty of 35-year-olds who do not. The difference is rarely the birth certificate. It is usually a combination of how much they move, how strong they are, how they recover, and whether small problems get addressed before they become big ones.
"You are just getting older" is not a diagnosis. It is a reason to stop looking.
If your knee hurts and the other knee, which is exactly the same age, feels fine, age is probably not the whole explanation.
Myth #4: Where It Hurts Is Where the Problem Is
Sometimes it is. Often it is not.
Low back pain can be driven by stiff hips. Knee pain can trace back to the ankle or the hip. Shoulder pain can start with a mid-back that no longer rotates. Headaches frequently come from the upper neck.
The body works as a chain. When one link stops doing its job, a neighbor picks up the slack and eventually complains about it.
This is one reason we evaluate the whole system rather than chasing the sore spot. Treating only where it hurts can bring short-term relief while the real driver keeps working in the background.

Myth #5: The Only Options Are Pills, Shots, or Surgery
Medications, injections, and surgery all have their place. Some situations genuinely need them, and we refer patients out when that is the case.
But a lot of people arrive at those options because nobody offered them anything in between.

If I had to boil it down, the patients who do best with pain tend to share a few habits.
They stay curious. They want to understand why something hurts, not just make it stop for the afternoon.
They keep moving. Not recklessly. Progressively. A little more each week.
They build strength. Strong tissue tolerates load better, and life is full of load.
They protect their sleep. Poor sleep lowers your pain threshold. Fixing sleep is one of the most underrated pain strategies there is.
They get help early. A nagging problem at week two is usually much easier to resolve than the same problem at year two.
When Pain Needs Prompt Attention
Most aches and pains are not emergencies, but some symptoms deserve to be checked right away:
- Pain after a significant fall, accident, or impact
- Numbness, tingling, or weakness spreading into an arm or leg
- Loss of bladder or bowel control
- Pain with fever, unexplained weight loss, or night sweats
- Pain that is severe and constant regardless of position
