Round Ligament Pain in Pregnancy: Sharp, Sudden and Usually Normal

You stand up a little too quickly, or you sneeze, or you roll over in bed, and suddenly there is a sharp, stabbing pain low in your belly or groin.

It stops you in your tracks. And if it is your first pregnancy, it can be genuinely scary.

In most cases, that sudden jab is round ligament pain. It is one of the most common discomforts of pregnancy, especially in the second trimester, and while it can be startling, it is usually completely normal.

What the Round Ligaments Do

Your uterus is supported by several ligaments. Two of them, the round ligaments, run from the upper sides of the uterus down through the groin on each side.

Think of them like supportive cords or guy wires that help hold the uterus in place.

Before pregnancy, they are short and relatively relaxed. As your uterus grows, they stretch and thicken to support the extra weight. Usually that happens quietly in the background. But when the ligaments are stretched suddenly, they can contract or spasm, and that is when you feel it.

What It Feels Like

  • A sudden, sharp, or stabbing pain on one or both sides of the lower belly or groin
  • Pain triggered by quick movements, standing up, rolling over, coughing, sneezing, or laughing
  • Pain that usually lasts only a few seconds
  • Sometimes a dull ache afterward, especially after a busy day on your feet
  • More often felt on the right side, though either side is common

It typically appears in the second trimester, when the uterus is growing quickly, but it can show up any time after the first trimester.

Why Some Moms Get It More Than Others

Every pregnancy is different, but a few factors seem to make round ligament pain more noticeable:

  • Pelvic imbalance. When the pelvis and sacrum are not moving evenly, tension on the uterine ligaments can become uneven too.
  • Quick, asymmetrical movements like twisting or getting out of the car one leg at a time
  • Long periods of standing or very active days
  • Subsequent pregnancies, when the abdominal wall offers a little less support
  • Carrying a toddler on one hip, which pulls the pelvis out of balance

Simple Ways to Ease It

  • Move slowly and deliberately, especially when standing up or changing positions.
  • Brace before a sneeze or cough. Bend your hips and knees slightly, or flex toward the side that hurts. Many moms find this helps a lot.
  • Roll over in bed with your knees together, moving your hips and shoulders as one unit.
  • Sleep on your side with a pillow between your knees and one under your belly for support.
  • Try a support belt during longer periods of standing or walking.
  • Use gentle warmth, such as a warm bath or warm compress. Avoid very hot baths.
  • Rest when you can. If the ache builds through the day, it is a signal to take a break.
  • Gentle stretching, like a cat-cow on hands and knees, can relieve tension for many moms.

It is tempting to move less when quick movements hurt. But gentle activity, like walking and prenatal-appropriate exercise, usually helps your body adapt and supports the muscles that stabilize the pelvis.

Just move with a little more intention and give your body time to transition between positions.

When It Is Not Round Ligament Pain

Round ligament pain is brief and tied to movement. Call your OB or midwife promptly if you notice:

  • Pain that is severe, persistent, or does not ease with rest
  • Bleeding, fluid leaking, or unusual discharge
  • Fever or chills
  • Pain or burning with urination
  • Regular contractions or tightening, especially before 37 weeks
  • Pain with nausea, vomiting, dizziness, or fainting

Is round ligament pain worse in a second pregnancy?

Many moms say they notice it earlier or more often in later pregnancies. The abdominal wall offers a little less support, and chasing a toddler adds lots of quick, twisting movements.

For most moms, it is most noticeable in the second trimester and often eases later as the body adapts, though some feel it on and off until delivery.

Can I keep exercising with round ligament pain?

Usually, yes. Walking, prenatal yoga, and swimming are generally well tolerated. Just move slowly when changing positions and ease off anything that triggers sharp pain.

A Normal Sign of a Growing Baby

Round ligament pain is uncomfortable, but it is usually just a sign that your body is doing exactly what it should, making room for your baby.
