The Third Trimester: Chiropractic Comfort in the Final Weeks of Pregnancy

The third trimester is its own experience.

The first trimester was exhausting. The second was, for most women, the comfortable stretch where things felt manageable.

Then the third arrives.

Suddenly you cannot find a comfortable position to sleep, your low back is talking to you all day, your ribs feel crowded, and getting off the couch has become a two-step process.

You are also being told, cheerfully and constantly, that this is just how it goes.

Some of it is. But a meaningful amount of third trimester discomfort is mechanical, and mechanical problems have mechanical solutions.

What Your Body Is Doing Right Now

The changes in these final weeks are dramatic, and they happen fast.

  • Your center of gravity has shifted forward significantly. Your low back compensates by increasing its curve, which loads the joints and muscles differently than they are used to.

  • Relaxin is circulating. This hormone loosens ligaments to prepare the pelvis for birth, which is essential but also means less passive joint stability everywhere.

  • Your pelvis is under new stress. The sacroiliac joints and pubic symphysis take on load they never carried before.

  • Your ribs and diaphragm are compressed. Which is why deep breaths feel harder and mid-back tightness is so common.

  • Abdominal muscles are stretched. Reducing the front-side support your spine normally relies on.

Put all of that together and the aching makes complete sense. Your body is doing something extraordinary under conditions it has never encountered.

Common Third Trimester Complaints

  • Low back pain, by far the most common

  • Pelvic girdle pain, often felt at the back of the pelvis or the pubic bone

  • Sciatic-type pain running down the back of a leg

  • Mid-back and rib discomfort

  • Round ligament pain in the lower abdomen

  • Difficulty finding any sleeping position that works

  • Swelling in the feet and ankles

None of these are things you simply have to tolerate for eight more weeks.

Is It Comfortable This Far Along?

Yes, and this is the question every third trimester patient asks first.

We use pregnancy-specific tables and supports that accommodate your belly, so you are not being asked to lie face down on a flat surface.

Techniques are modified and gentle. There is no aggressive twisting of the abdomen or pelvis.

Positioning is adjusted to whatever is comfortable for you on that particular day, which can change week to week.

Most patients tell us the visit itself is one of the more comfortable parts of their week.

Practical Comfort Strategies

Sleeping

  • Side lying, generally on the left, is typically recommended in later pregnancy

  • A pillow between the knees keeps the pelvis more level and reduces pull on the low back

  • A pillow or wedge supporting the belly reduces the drag on your abdomen

  • A small rolled towel at your waist can support the side-lying curve of your spine

Sitting

  • Feet flat, hips slightly above knees if you can manage it

  • Support behind the low back

  • Get up and move every thirty minutes or so

  • Avoid crossing your legs, which pulls the pelvis out of level

Standing and moving

  • Avoid standing with all your weight on one hip, which is a very natural habit late in pregnancy

  • Turn your whole body instead of twisting at the waist

  • Get out of bed by rolling to your side first, then pushing up with your arms

  • Supportive shoes matter more now than they ever have

Daily movement

Walking, prenatal yoga, and swimming are all excellent if your provider has cleared them.

Swimming in particular is wonderful in the third trimester, because the water unloads your joints entirely for a while.

The Pelvic Floor Deserves Attention

This gets overlooked constantly.

Your pelvic floor is under sustained load right now, and it plays a role in both birth and recovery afterward.

It needs to be able to both contract and relax. Many women are taught only about tightening.

If you have not talked with anyone about pelvic floor function, it is worth raising with your provider or working with a pelvic floor physical therapist.

Looking Ahead to Recovery

The care you receive in these final weeks does not stop mattering when the baby arrives.

Postpartum brings its own mechanical demands: feeding positions, carrying a growing infant, car seats, and a body recovering from an enormous event.

Women who go into that period with a well-functioning spine and pelvis tend to have an easier time with it.

And when you are ready, we adjust babies too. Our pediatric care is gentle and appropriate for even the newest patients.

We work alongside your OB, midwife, and the rest of your care team, not in place of them.

Please tell us about any complications, restrictions, or concerns your provider has raised. We want the full picture, and we will say so if something is outside our scope.
